MORONGO BASIN SPAY AND NEUTER

Please read and acknowledge before completing this document:

We do not perform surgery on female dogs over 80 pounds and male dogs over 90 pounds 
All pets must be 2 months old and weigh 2 lbs.
Male dogs must have two testicles.
We do not do any brachycephalic breed (flat faced) pets (bulldogs, french bulldogs, pugs). 

We do not do any Dobermans due to Von Willebrand disease 
No pets over 6 years old (requires pre-anesthetic bloodwork)
We will not do pets with heart murmurs, any seizures/neurological history and/or
females with prior abdominal surgeries.
